For me, it would be cost. The best, most comprehensive healthcare service would be of little use to me if eg, it cost more than my own salary. Any service I use will be a compromise between cost and other factors like being professional. The only thing I cannot accept is not being safe.

I work at a long term care facility in a state funded hospital. We will accept nearly anyone regardless of their assets including the homeless and people who have never done paid work, because the government will pay us something through Medicare or Medicaid for our services. Our system operates at a substantial loss every year and our state does not provide adequate funding.
